Payment Options

We accept checks, cash, money orders or credit cards. In some cases, if payment is made in full at the time of treatment, a 5% discount will be applied. Dental Insurance

Insurance is an agreement between your employer or you and the insurance company. We believe that insurance coverage should not dictate our dental treatment. Your dental care should be an agreement between you and your dentist. We accept many benefit plans and if you have dental benefits we would be happy to file your claims for you. If you are considering new or different insurance benefit plans, please call our office manager to discuss the pros and cons of that plan. Be sure to choose a plan that allows you to choose your own dentist.

Patient Forms

If you are a new patient to our office, please visit our patient forms page to download our new patient forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.
